Need a nursing diagnosis for care plan? Please help!?
I am in nursing school and I am making a care plan.. I have to have 3 nursing diagnosis for my patient. My patient is nonverbal, immobile, has a g-tube. I have Skin breakdown and impaired physical mobility. My 3rd one was risk for aspiration but the teacher said not to use any "Risk For" diagnosis. I was going to use Powerlessness because this patient cannot care for herself but I'm not sure of a goal for something like this because they are unable to express anything. Can someone please help??

alteration in nutrition, fluid volume deficit,potential for infection, self care deficit, impaired verbal communication, and you can use a goal that they don't get worse.
