Is there something wrong with my piercing?

I got my labret pierced 4 weeks ago…some people told me it should be pretty close to being healed by now, but it doesnt feel like it. Ive done the sea salt soaks, ive washed my mouth out, ,kept my hands clean and everything, but it feels like theres something wrong with it. I know my body's not rejecting it because it was perfectly fine for a while but one day my ball came off my ring so i tugged on it for like an hour trying to get it back on because i didnt want my ring to fall out while i was sleeping..it swelled up for a while after that and now its very sensitive and it feels like there's a lump on the inside of my lip right underneath the piercing…its not dead skin because the dead skin already fell off, it feels like a knot or like there's a bead or something underneath my skin. im just confused because it looks fine on the outside and it only has crusties on it when i first wake up…so could it be infected? i need any info i can get about what it could be and how i can take care of/ handle it.
