Good facial lotion for very dry skin… also pore minimizer?

My facial skin is horribly dry… its noticeably flaky and red (not real red but more pink on my cheeks and chin). I spent a fair amount of money on some Clinique skin care facial wash and lotion that was suppose to help but it doesn't work. I have been using it for months. The lotion helps make the dryness less visible but doesn't treat the overall problem.

Can anyone recommend a good lotion that will make my skin smooth again? I am very self conscious about my face… its even hard to get makeup to cover it up. I barely ever wear make up… and when I do, doesn't hide my dry skin.

Also, I seem to have large pores… can anyone recommend something for that? Thanks so much!!!
